What is botulinum toxin treatment?

Botulinum toxin is a neurotoxin that inhibits the release of the neurotransmitter acetylcholine at the level of the neuromuscular junction. In other words, muscle contraction is temporarily blocked, so dynamic wrinkles are corrected. The mechanism is reversible and only works for a limited time.

Following treatment with botulinum toxin, the effect of attenuation or erasure (for a limited period of time) of dynamic wrinkles is observed. In addition, it is also used to prevent the appearance of wrinkles. Moreover, treatment with botulinum toxin can positively change the general appearance of the face – by giving it a more relaxed and optimistic appearance, an effect achieved by relaxing the depressor muscles.

After the injection, the effect sets in 3-4 days and is maximal around days 10-14 post-intervention.

The duration of the clinical effect, which lasts between 3 and 5 months, depends on the doses. Furthermore, individual patient variations matter – in other words, how long it takes the body to regenerate neuromuscular synapses. That is, to return the muscle to the initial situation, before the botulinum toxin injection.

Regeneration sets in gradually, the fibres gradually resuming their activity, as the clinical results diminish. The improvement in the appearance of wrinkles observed over time is also due to a special mechanism – it deposits collagen in the relaxed area by injection.

Precautions – contraindications

Botulinum toxin injection is not recommended for people who:

  • Are going through pregnancy or breastfeeding;
  • Have local infections in the injection area;
  • Have hypersensitivity reactions to botulinum toxin;
  • Have neuromuscular diseases (myasthenia gravis, multiple sclerosis);
  • Are over 65 years old – this contraindication is due to the lack of an adequate clinical response;
  • Are allergic to albumin or human source;
  • Are following a treatment with Aspirin, Aspenter, Plavix, Clexane, Sintrom or another anticoagulant/antiplatelet agent in the last two weeks (these create an increased risk of bruising).

Possible side effects

Following botulinum toxin injections, there is the possibility of adverse reactions such as:

  • Edema (swelling of the area) and erythema (redness of the skin) in the injected area;
  • Bleeding and ecchymoses (bruises);
  • Allergic reactions;
  • Upper eyelid ptosis, eyebrow ptosis, eyebrow asymmetry, oral commissure asymmetry, diplopia, headache, exaggerated muscle weakness.

Recommendations after botulinum toxin injection

Following botulinum toxin treatments, it is recommended:

  • To avoid exercise and sleep for at least 4 hours after the injections, so as to avoid the migration of the substance;
  • To avoid facial treatments involving massage for 2 weeks post-treatment;
  • To avoid aspirin for the first 7-15 days before and 7-15 days after the botulinum toxin injection, to minimise the risk of bruising;
  • In addition, it is forbidden to perform local massages in the first 24 hours after the injection, in order to limit the risk of migration of the substance to other muscles.
