Acne

Acne is an inflammatory, chronic, multifactorial disease that affects the pilo-sebaceous unit. It appears in areas where the density of sebaceous glands is high: face, back, décolleté.

The mechanism by which the acne lesion is produced are:

  • Increased profiling of the epithelium lining the follicle – ultimately leads to the blockage of the pore.
  • Increased production of sebum.
  • The presence of a commensal bacterium, Cutibacterium acnes, which normally lives on the skin – it degrades sebum into potentially irritating fatty acids, giving birth to acne.
  • Inflammation.

Factors that influence the appearance of acne:

  • Genetic factors: the number and activity of the sebaceous glands are inherited.
  • Hormonal changes: polycystic ovary syndrome, puberty, pregnancy.
  • Certain medications.
  • Certain cosmetic products with a comedogenic effect.
  • The use of tight clothes that lead to the blocking of pores.
  • Stress.

Diet is also a factor that seems to influence/accentuate acne lesions in predisposed individuals. The foods most frequently incriminated are those with a high glycemic index: milk, chocolate (and sweets in general – high glycemic index), diets rich in fats (poor in omega 3).

Hormonal acne is a form of acne that appears even after adolescence, which usually doesn’t respond that well to treatment and is accompanied by frequent relapses. It usually appears in adult women, frequently along the mandibular line, the appearance of lesions being influenced by hormonal fluctuations. The lesions are often deep, painful and can result in the formation of scars/erythema/post acne hyperpigmentation.

The treatment is complex combining topical preparations, systemic treatment (including hormonal) and various procedures (Gold Toning, Hollywood peel, Dermalinfusion, Laser CO2). It is also important to find a team of doctors (gynaecologist, dermatologist) that you can trust and with whom you can manage this condition.

Types of acne lesions:

  • Non-inflammatory: closed (whiteheads) or open (blackheads) comedones.
  • Inflammatory: papules, pustules, nodules, cysts.
